Procedure to apply for CGHS Cards Online for Pensioners and Serving Employees

Eligible serving employees/pensioners have to apply online. However at present, provision for uploading the supporting documents is not available. After online submission of the application, the applicant is required to take a print out of the application and submit it with supporting documents (duly endorsed by the Head of the Department/Office in case of serving employees) to the office of the Additional Director, CGHS Mumbai. The form should be completely filled up with individual photographs pasted as specified in the form.

Following documents as applicable need to be attached :

In case of Serving employee:

  1. Duly filled up online application form with forwarding note, signature, and stamp of the Officer in Charge. The signature and stamp of the forwarding officer should be there on each page of the form.
  2. Proof of residence.
  3. Two passport size individual photographs of all family members of the serving employee, one set of which shall be pasted on the application form.
  4. In case of differently abled dependent son (above 25 years) or brother (above 18years) disability certificate (more than 40%) from competent authority as specified.
  5. Documents for proof of age for all applicants. Birth certificate for the following applicants: Son (up to 25 years), Brother (up to 18 years).
  6. Legal documents for widowed or divorced daughter.
  7. Latest payslip with signature and stamp of the Officer in Charge.
  8. An undertaking by the employees with the approval and forwarding note from the Officer in Charge who are residing beyond the coverage area of CGHS WC but within the same Municipal limits.
  9. A copy of the transfer note by the office of the Additional Director of the city from where he got transferred, transfer order, joining report in the new place, address proof and the latest payslip of the new station for the employees who have been transferred from other cities.

In case of Pensioners:

  1. A printout of the duly filled up online Application Form by visiting the website cghs.nic.in
  2. Copy of the PPO.
  3. Copy of the LPC (Last Pay Certificate).
  4. Proof of residence.
  5. Documents for proof of age for all applicants. Birth certificate for the following applicants: Son (up to 25 years), Brother (up to 18 years).
  6. Legal documents for widowed or divorced daughter.
  7. Aadhar card copy.
  8. Surrender certificate of CGHS Card (only if CGHS Card was issued during Service Period).
  9. Requisition for Pensioner’s CGHS card(s) shall be accompanied by three copies of recent 3 x 5 cm size individual photographs of all family members of the retiring employee, one set of which shall be pasted on the application form and shall be attested by a Gazetted Officer (other than CGHS).
  10. Certificate regarding Fixed Medical Allowance (FMA) should be mentioned in the PPO/ LPC/ or recent certificates should be obtained from the bank. In case the details of Fixed Medical Allowance is not mentioned in the PPO, a certificate from the HOD/ Registrar from Court regarding non-drawal of Fixed Medical Allowance should be attached (template).

The amount for payment through Bharat Kosh/ Demand Draft will be the contribution due for one year if the card is to be made annually and for 10 years if whole life card is required.

Channel of Submission: In case of serving employees the application is to be submitted through the department, duly sponsored by the sponsoring authority.

In case of Pensioners, the application with relevant documents and enclosures are to be submitted to the Office of the Additional Director, Mumbai or the nearest CGHS Wellness Centre.

The CMO In-charge of the CGHS Wellness Centre shall verify all the documents against original and forward it with remarks through the messenger to the office of the Additional Director every week.

Dependency Criteria: For availing the medical facilities under the scheme, parents (or parents-in -law in case of female employee), unmarried son till 25 years of age, dependent unmarried/widowed/divorced/separated daughters and sisters, minor brothers are deemed dependent on the Government employee if they are normally residing- with him and their income from all sources including pension and pension equivalent of DCRG benefit is less than Rs. 9000 + DA per month. This criteria does not apply to spouse and disabled son irrespective of age (please see definition of disability in instructions for filling CGHS card).
