Documents required are:
- Request letter addressed to the Additional Director, Mumbai for purchase of CAPD fluids.
- Advise of Government nephrologist recommending CAPD and indicating the consumables (CAPD fluids and Y SET, Transfer set, Mini cap etc) required, along with the number(amount) of each item required every month.
- Other relevant medical documents of beneficiary illness.
The permission is accorded for a period of six months. In case of serving employees, the permission shall be obtained from their parent department.
